Enrollment is voluntary, is not a condition of employment or of any service, and may be declined or revoked at any time. This page documents the opt-in experience presented to employees.

This is the exact opt-in step presented to employees in the web enrollment flow.




The employee may skip this step entirely and complete onboarding with full access. The checkbox is never pre-selected.

Paper Form Opt-In

Employers may alternatively offer a standalone printed enrollment form, separate from required onboarding paperwork, containing the identical disclosure language above. The signed form is retained by the employer solely as the record of voluntary consent. Declining carries no consequence.
